STEP #2: COMPLETE SKILLS CHECKLIST (complete only those for your occupation and specialty). All nurses must complete the Essential Nursing checklist and checklist for the specialty(ies) for which they would like to be considered (if shown).

STEP #3: SUBMIT CONSENTS AND AUTHORIZATIONS

• Professional/Clinical Reference #1
• Professional/Clinical Refernce #2
• Professional/Clinical Reference #3 (often required by clients)
• Background Check Authorization
• Background Check Registration (Registration through VerifiedCredentials.com)
• Medical Release Authorization
• Substance Abuse Testing
• Child Abuse Policy

• Varicella Attestation
• MMR History/Declination
• Hepatitis B: Verification or Declination (Complete One)
• Latex Allergy Release
• Tetanus and Diptheria Toxoid
• Influenza Vaccine Acceptance/Declination
• Emergency Contact Form

STEP #5: EMAIL US SCANNED IMAGES OF YOUR DOCUMENTS

• Email Scanned Documents: If you have not already included scanned copies of the following documents when completing any of the forms listed above, you can send these to us via email. Common formats are .pdf, .jpg., .gif.

STEP #6: COMPLETE TESTING

• You will receive an email from NurseTesting.com with links to online competency and OSHA testing.

STEP #7: COMPLETE CONTRACT PROFESSIONAL AGREEMENT

• Upon acceptance of a contract assignment and prior to commencing assignment you must complete our Contract Professional Agreement.
